Please also bring your insurance information if you have coverage and any recent X-rays if they are available. What happens at the first visit sets expectations for your consultation.
Why we ask for it in advance
Your health history includes medical conditions, medications, allergies, and any previous dental or orthodontic treatment. This information helps the doctors understand anything that may affect tooth movement, gum health, or treatment planning.
Completing the form before you arrive allows more of your first visit to focus on the exam, conversation, and next steps. If anything changes during treatment, such as a new medication or diagnosis, let the office know so your records stay up to date.
If you prefer to complete the form in the office, that is fine too. Please arrive a little early, and our team will help you get started.
